Youth Hospital in The Home referrals

The Youth Hospital in The Home (Y-HiTH) service provides acute, hospital standard mental health care at home for young people aged 16 – 24 as an alternative to going to or staying in hospital.

The service acts as a step between being admitted to hospital and community care.

Eligibility

Young people aged between 16–24 years living in the South Metropolitan Health Service mental health catchment area (PDF 166KB) who are experiencing an acute episode of mental illness and require hospital admission are eligible for this program.

To participate, the young person must be willing to receive treatment at home rather than in the hospital and agree to daily home visits.

The young person will need stable accommodation that is safe for the team to visit. It is preferable that the young person has family, friends, or carers who are willing to be actively involved during this period.

Referring a patient

All referrals are to be discussed with the Y-HiTH Nurse Unit Manager by phoning the HelpDesk on 6152 2222.

WA Health staff referring patients are requested to submit a referral via eReferral.

External health professionals are asked to complete the attached referral form (Word 85KB) and email to FSH.MHYouthHiTHReferrals@health.wa.gov.au
